For chronic patients: increase the access to chronic and sometimes vital treatments: wherever they are and whenever they need it, even if the Family Doctor or the Specialist is not available to re-prescribe it:

The Challenge

Lack of medical adherence to treatment for chronic patients due to unavailability prescription with all the consequences it might lead to -Save doctors time (GP's and Specialists) -Facilitate the work of Pharmacists ( no need of inventory management) -Some pharmacists deliver chronic treatment without medical prescription because they know that the patient used to take this treatment and tell the patient to come later with the prescription (illegal, can be dangerous if the treatment has changed in between

The solution

An Application on the patient's mobile phone. Digital Loggin via ID Tokken All the chronique treatments appear (list of chronic drugs predetermined and validated together with the Doctor) -Click on the drug(s) that needs to be re-prescribed -Choose the pharmacy you want your drugs to be delivered at (Google-Map opens up to select a pharmacy or the name of a pharmacy already recorded) -Payement: choose on line (can choose only if pharmacy already known )vs at the pharmacy -an electronic prescription is generated -At the same time, PharmApp commands the medicine at a wholesaler where it is available, and who will deliver it directly at the pharmacist's -The patient is informed by PharmApp that the delivery is available at the pharmacy -The pharmacist scan a QR code joined with the delivery: it gives direct access to the electronic prescription -The pharmacist can deliver the treatment and see if payement is still needed -After x number of electronic prescriptions (defined by the Doctor: 3month, 6 month, 1 year...) the patient has to see de Doctor again if he wants to go on with this automated process of prescriptions (to reactivate it) and so that the Doctor sees if the same therapy is still needed, if the dosis needs ajustment etc..**/
